Grant Report Form

Organizations and individuals receiving grants from the Minnesota Credit Union Foundation are required to submit a follow-up report. To receive feedback from organizations supported by the Foundation and to help fulfill IRS reporting requirements, please submit the appropriate form within one month of completion of the event or project funded.

 

Completed forms can be submitted via email to the Minnesota Credit Union Foundation or mailed to the attention of the Foundation at 555 Wabasha Street North, Suite #200, St. Paul, MN 55102.
